Geriatric Care Management:
What is a Professional Geriatric Care Manager?
A Professional Geriatric Care Manager (PGCM) is a health and human services specialist who serves as a guide, advocate, and resource for families caring for older relatives. A PGCM assists older adults and persons with disabilities to reach their maximum functional potential. The individual’s independence is encouraged, while safety and security concerns are also addressed.
A Professional Geriatric Care Manager (PGCM) is a member of the National Association of Professional Geriatric Care Managers (GCM) and is certified and committed to adhering to the GCM Pledge of Ethics and
Standards of Practice.

What Can a PGCM do for you?
- Conduct care planning assessments to identify problems and to provide solutions
- Screen, arrange, and monitor in-home help or other services
- Provide short- or long-term assistance for caregivers living near or far away
- Review financial, legal, or medical issues and offer referrals to geriatric specialists
- Provide crisis intervention
- Act as a liaison to families at a distance, overseeing care, and quickly alerting families to problems
- Accompany to appointments
- Assist with moving an older person to or from a retirement complex, assisted care home, or nursing home
- Provide consumer education and advocacy
- Offer counseling and support
Medication Set Up & Monitoring:
Our caring nurses can provide weekly, monthly, or as needed assistance with setting up medication dispensers, verification of medication compliance, review of medications with physicians, help with re-fills.
Daily Money Managing:
When managing the household bills and paperwork becomes too confusing or unmanageable, we can assist with Daily Money Managing tasks. Our support includes:
- Bill paying - preparation of checks for client signature, reconciling checking accounts and investment statements, preparing and making bank deposits
- Organizing tax documents
- Negotiating with creditors
- Reviewing medical insurance papers, and verifying proper processing of claims
- General organizational/filing assistance
Guardianship
Guardianship is a legal process utilized when a person can no longer make or communicate safe or sound decisions about his/her person and/or property or has become susceptible to fraud or undue influence. Because establishing a guardianship may remove considerable rights from an individual, it should only be considered after alternatives to guardianship have proven ineffective or are unavailable. Friendly Visits & Calls
A daily, weekly or monthly friendly visit from one of our caring
professionals can help to monitor general home and personal conditions.
While providing socialization and companionship, we are continuously
monitoring for changes in condition and problems. This service may be
helpful when a caregiver needs to go out of town or for long distance
caregivers. We can provide feedback via email or phone as to the
conditions found and any needs noted.
A daily or weekly telephone call can provide a helpful "check in" from a
caring supporter. We can also help with reminders to take medicine or keep
appointments.
